Youth Support Program (YSP)
Our Youth Support Program offers a voluntary, free, and confidential service to young people aged 12-21 who are at risk of:
- Disconnecting from family, community, or support networks
- Disengaging from school, training, or employment
- Experiencing harm, including self-harm
- Facing homelessness

Services Offered
Support and Case Management
Our Case Management Services provide coordinated and integrated responses to young people with complex needs, ensuring they receive comprehensive support across multiple service providers.
Key Components of Case Management Support:
- Conducting thorough needs and strengths assessments, including ongoing risk assessments and safety planning.
- Collaborating with young people to create support plans that strengthen their support networks, including family involvement, while enhancing their skills and resilience.
- Offering practical assistance, advocacy, and interpersonal support as needed, including safety planning.
- Facilitating referrals to specialist services, including drug and alcohol support, mental health care, housing assistance, legal services, and domestic violence support, in addition to general health services.
- Collaborating with other service providers to effectively address the needs of young people.
- Partnering with agencies to enhance local service delivery.
Program Outcomes
The Youth Support Model focuses on achieving four key outcomes for young people:
- Fostering positive relationships with family and community
- Engaging in education, training, or employment
- Leading healthy, violence-free lives
- Securing safe and stable living environments
Referrals
To refer a young person to our program, please download and complete the Referral to YSP along with the Consent to refer form.
Important: We cannot process referrals without a completed and signed Client Consent to Referral section by the client and/or their guardian.
